Welcome to Evolve.
After you submit your appointment request, a caring staff member will call you within one week to confirm next steps. If you'd prefer to speak with someone sooner, please call our patient care coordinator directly.
Established Patients
Welcome home.
If you're already established with our office, please note that we are no longer using Healow. Please use the Elation Patient Passport (patient portal) for non-urgent communication and scheduling requests, call our patient care coordinator directly.

Appointment policies.
These policies help us protect our patients, our staff, and the valuable time of everyone involved in your care. Please review them before requesting your visit.
Pay-as-you-go patients. Those who miss their appointment without notice are subject to a $50 no-show fee.
Repeated no-shows. New patients who miss their first appointment without notice will not be permitted to reschedule. Established patients who no-show more than once will be discharged from the practice.
A respectful environment. We have a zero-tolerance policy for abuse of any kind toward our patients or staff.
