    Evolve Gynecology operates outside of insurance billing, which means Dr. Hodges cares for a smaller patient panel and can give each patient the time, attention, and access they deserve. Membership is your direct relationship with your provider — not just a visit.

The Whole Health Membership combines full concierge gynecologic care with direct primary care access through our Physician Assistant — one practice, two providers, comprehensive coverage. Ideal for patients who want a trusted care team for both their gynecologic and everyday medical needs.

Billing

Insurance and Billing

Evolve Gynecology does not bill insurance for office visits. Members may continue to use insurance for eligible services such as laboratory testing, medications, outside imaging, and specialist referrals when applicable.


We encourage you to inquire with your insurance about requesting reimbursement with your out of network benefits and your HSA or FSA account.


Self pay laboratory discounts are available through Clinical Pathology Laboratories, CPL.

What Is Not Included

Laboratory testing, medications, outside imaging, and specialist referrals are not included in the membership fee. Insurance may be used when applicable.


Office procedures are not included and are offered on a direct pay basis only (Concierge members get a 10% discount on office procedures). These services are not billed to insurance.

  • Why does Evolve operate outside of insurance?

    Insurance requirements limit visit time, restrict care decisions, and create administrative friction. Operating outside of insurance allows Evolve to focus entirely on personalized, patient-centered gynecology — where care is driven by need, not by coverage rules.

  • Am I able to use my insurance for any other services?

    Yes. Insurance may still be used for labs, medication, imaging, and specialist referrals when applicable. We encourage you to inquire with your insurance about requesting reimbursement with your out of network benefits and your HSA or FSA account.

  • What does the membership include?
